A clinical analysis of traumatic splenie injury

Abstract
|
|
|
This analysis was carried out to evaluate the problems of traumatic splenic ruptures(30 cases) which were performed in St. Mary¢¥s Hospital, Catholic Medical College, during last 5 years from April 1967 to March 1972.
The results were summarized as follows:
1) The traumatic rupture was most frequently caused by traffic accident (46.2%0) followed by fall accident and kick injury. The majorities (86.75vo) of patients were between 1st and_ 4th decades of life.
2) In symptoms and signs, the¢¥generalized.abd61ninal pain(89.9%) and tenderness on Lt upper quad-rant(83.3%6) were most common;
3) In lab data, 53.3% of patient was noticed leucocytosis over 15, 000/mm3 and diagnostic pdra-centesis was positive in 93.2%.
4) The characteristic findings in flat abdomen x-ray was about 48%.
5) The most common type of splenic injury was fragmentation of splenic parenchyme(56. 8%),
.and frequent associated injuries were the rib fractures(26. 6%) and liver lacerations(20%).
6) The incidence of postoperative complications was 46.6% and most common complications were
pulmonary complications and subphrenic abscess.
7) The 3 cases were expired due to multiple associated injuries and postoperative enterocutaneous fistula.
